Incident Lesson Learned in this element of Process Safety Management from PEMEX LPG Terminal 1984.

The PEMEX LPG Terminal incident in 1984 serves as a tragic reminder of the importance of process safety management. A major fire and a series of catastrophic explosions occurred, killing 500 individuals and destroying the terminal. The incident was caused by a ruptured 8-inch pipe, which released LPG for about 5-10 minutes before drifting to a flare stack and igniting. This led to a series of BLEVEs and ground fires over the next hour and a half.
