LEARNING FROM ACCIDENT : HUSKY REFINERY FIRE AND EXPLOSION APRIL 26th, 2018

Husky Energy Refinery explosions occurred on April 26th 2018, with 36 employees injury and major asset & reputation damage.

The accident occurred during the refineries fluid catalytic cracker (FCC) shutdown for periodic maintenance. A slide valves, which meant to separate the flammable hydrocarbon part of the cracker and the air-filled regenerator part, was heavily eroded. It caused a mixture of flammable hydrocarbon and air, which finally ignited and exploded.

The debris from the explosion hit the nearby asphalt tank storage, causing asphalt leakage around the area. The asphalt was also finally ignited. Fortunately, the fire can be controlled within hours with great effort from the company`s and local fire fighter.

There was an inadequacy of Safety and Critical Element (SCE) identification and maintenance. The FCC slide valve should have been an important SCE barrier and maintained accordingly
